One per line. They print on page 2 with space to write the answers.
What the two pages hold
| Page 1: your numbers | Page 2: the conversation |

| For each measure the visit type covers: the latest value and date, the median of the last 30 days with its lowest and highest day, the median of the 60 days before, and a small chart of every day across 90 days. Each device gets its own row. | Your medication list, taken from medication records in your export and edited by you. Your questions, numbered, with lines for the answers. A page for notes from the appointment. |
- Your values, not a verdictThe PDF shows what your devices recorded. It has no reference ranges, no scores and no interpretation, and it does not diagnose anything.
- Devices kept apartTwo devices measuring the same thing get two rows, never an average. Steps use the highest single-device total each day, because iPhone and Apple Watch count the same steps.
- Nothing uploadedThe file is read by JavaScript on this page. The page sends nothing anywhere and runs no analytics, so it works offline once loaded.
- Medications from your recordsMedication records your health provider shared with Apple Health come from export.zip. Anything missing, you type in; the list goes only into your PDF.
Questions, answered straight.
- Is my health data uploaded anywhere?
- No. Your browser reads the file on your device and makes the PDF on your device. Nothing is sent to us or anyone else.
- Which file do I need?
- The export.zip from the Health app on your iPhone (your picture, then Export All Health Data). Drop the zip itself to include medication records; export.xml alone works for everything else.
- My export is huge. Will it work?
- The tool reads the file as a stream and keeps only the last 90 days of the measures it needs, so years of history are fine on a computer. Very large zips (over 4 GB) need unzipping first; then drop export.xml.
- Why does a measure appear twice?
- Two devices recorded it, for example a watch and a ring. Each keeps its own row so your doctor sees both, instead of an average that neither device measured.
- Does it tell me what my numbers mean?
- No. It organises your own values and trends into a document for the conversation. Your clinician reads them with you.
- Is this a medical device?
- No. It does not diagnose, treat or interpret anything.
